Make payment by wire transfer, certified check from U.S. bank, or Western
Union. Specific payment instructions are given in item 10 on the
form.
Instructions for filling out the form:
Corporation Name: Provide three
names, in order of preference. We will use the first name available,
in order of preference. If all three names have been taken,
we will inform you by e-mail.
Corporation Objective: Provide a quick
summary of what the corporation will be used for. This can be very
general, as in: "to provide retail sales of clothing" or "to invest
in corporate securities"; or "to purchase and hold real estate".
The Corporation Objective should indicate the
nature of the corporation's intended business activities. The
Corporation Objective does not limit the Corporation from doing
business in any other field, however.
Authorized Capital: This amount is
usually US$10,000.00, which does not have to be paid in. Amounts above
US$10,000.00 are subject to a higher filing fee.
Authorized Shares: This is normally 100
shares. Additional shares can be authorized and issued at a later
date.
Type of Shares: Please indicate
whether the shares will be issued in bearer or named form, or both.
Power of Attorney: The Power of
Attorney is normally issued to the individual who will control or direct
the company, sign checks and bank accounts, etc. The individual can
be (but does not have to be) a member of the
Board of Directors or a stockholder. A General Power of
Attorney is issued to an individual, granting that individual complete
control over the company. A Special Power of Attorney grants an
individual specific powers to act in representation of the company, for
example, to open a bank account and sign checks, etc. If desired,
please indicate if you want a General or Special Power of Attorney issued.
Also indicate the name and passport number of the individual to hold the
Power of Attorney. This Power of Attorney will be included in the
Articles of Incorporation filed at the Public Registry. A separate
Power of Attorney (either General or Special) can be issued and registered
at a later date, if desired.
Board of Directors: If Board Members
are provided by you, provide names, nationalities, passport #'s, and
addresses of each. If Board Members are provided by us, leave this
section blank. The President, Secretary and Treasurer's names and
information will be included in the Articles of Incorporation filed at the
Public Registry.
Contact Person: This is the individual
to whom we will send the registered Articles of Incorporation, Certificate
of Good Standing, translations, etc., as well as any future
correspondence from our office. Provide name, address, phone, fax and e-mail.
The Contact Person can be (but does not have to be) a Director, stockholder, holder of
Power of Attorney, or Filer, and will not appear on any documents
registered at the Public Registry.
Payment Amount: Calculate and
total your payment amount.
Payment Method: We accept Wire Transfers, Certified Checks from U.S. Banks, and Western
Union.
Other Instructions: Any
questions, comments, other services needed, further instructions,
preferred method to contact you, etc.
Filer's Information: The Filer is the
individual who fills out this form. The Filer can be (but does not have to be) a Director, stockholder, holder of
Power of Attorney, or Contact Persons, and will not appear on any
documents registered at the Public Registry. Provide Filer's name,
signature and passport #. The Filer's information will not
appear on any documents registered at the Public Registry.
