Copy of passport photo page of applicant (& spouse & dependents, if applicable) that shows the picture, name, date of birth, passport number, place of issue, etc. Note: all passports submitted must be valid for at least six months from the time of application submission.

Six (6) passport sized photographs of the applicant (& spouse & dependents, if applicable) The applicant should be wearing a business suit.

Police Record of applicant, spouse (if applicable), and dependents over age 18 (if applicable) from locality where applicant has resided for previous two years. The police record should cover the previous five years, if possible, or state that the applicant (and spouse and dependents, if applicable) have no arrest record.

Copy of Marriage Certificate of applicant and spouse (if applicable), duly notarized and authenticated (by apostil from your state or national government, or by the Panamanian Consulate nearest you).

Birth Certificate of minor dependents (if applicable), duly notarized and authenticated (by apostil from your state or national government, or by the Panamanian Consulate nearest you).

the following corporate documentation from Panama: (if you do not have a corporation set up yet, we can form one for you.)

Notarized Certification Letter from company accountant, stating that the applicant has invested more than US$40,000.00 in the company (our CPA can provide this certification, if necessary).

Notarized Certification Letter from other company director (Secretary or Treasurer) attesting to the applicant's investment.

Notarized Copy of Stock Payment (copy of cancelled check, wire transfer receipt, etc.)

Notarized Copy of Stock Certificates issued to investor.

Notarized Copy of Panamanian Social Security payroll which demonstrates employment of at least three (3) Panamanians.

Copy of Business License (should be Type A).

Good Standing Certificate from Panamanian Social Security.

Corporate Tax Return (copy of first or most recent year's tax declaration, along with evidence of payment).

Good Standing Certificate for the applicant (issued by Panamanian tax authorities).

Individual Tax Return (copy of first or most recent year's tax declaration, along with evidence of payment).

The Signed Power of Attorney (contact us for a specific power of attorney).

The Signed Letter of Responsibility, wherein the applicant assumes financial responsibility for his/her spouse and/or dependents (this is not required if there are no spouse/dependents). Contact us for a specific letter of responsibility.

Make payment by Visa, MasterCard, wire transfer, certified check from U.S. bank, or Western Union. Specific payment instructions are given in item 19 on the form.
On your initial visit to Panama: visit our doctor for your Certificate of Good Health and our laboratory for your HIV test (these tests and certificates are included in our fee) for yourself and your spouse / dependents (if any).
On your initial visit to Panama: issue a certified check from a Panamanian bank made out to "Ministerio de Gobierno y Justicia" (Panama's Government and Justice Ministry) totaling US$500.00 each for investor, spouse and each dependent over 12 years of age (if applicable). This is a repatriation security bond, which is refunded to the investor after five years.
On your initial visit to Panama: issue a certified check from a Panamanian bank made out to "Tesoro Nacional" (Panama's National Treasury) totaling US$100.00 each for investor, spouse and each dependent regardless of age (if applicable). This is the government's filing fee for investor visas, and is non-refundable.

Please contact us prior to sending any documentation. For your records, remember to keep copies of all forms, documents, etc. sent to us.

Investor's Home Address: This is your street address in your country of current residence (this can be Panama, if you're living here).
Investor's Mailing Address: If different from your street address.
Investor's Passport Number.
Investor's Social Security Number.

Investor's E-mail Address: This is for our internal records, and will not be included in the Visa application.
Investor's Home Telephone Number: This is for our internal records, and will not be included in the Visa application.
